We offer physician services at several local extended care facilities. Our physicians make rounds on a regular basis and provide care oversight at each of the following medical facilities. If you would like to arrange admission to one of these facilities or transfer care at one of these facilities to our physicians, please contact the admissions director at the facility.

Medical Care questions
Our physicians make rounds at each facility on a scheduled basis. If you have questions about care of a patient in an extended care facility, please direct the question to the supervising nurse at the facility. He/she may be able to answer many of your questions. If there is a problem that requires physician orders or review they will contact our physician for instructions. We are available for daily contact with each facility either by phone or fax. The staff at our office is not able to provide you with any information about patient care, as the records are kept at the nursing facility.

Rehab Services
Many patients in extended care facilities are receiving rehabilitative care services such as physical therapy, occupational therapy or speech therapy. For questions regarding therapy progress, please contact the therapy department directly, as they will be able to give you the most accurate update.
 
Referrals
Referrals for care by other providers are ordered by the physician and managed by the facilities. If you have questions about a referral or transportation to a referral please contact the nursing staff at the facility.

Billing Services
For questions regarding billing for physician services at extended care facilities you may contact our billing manager at 717-636-3335. If you have questions regarding billing for facility services including medications, laboratory and radiology testing, please contact the billing department of the facility.

Discharge Planning
If you have questions about discharge planning for a patient at an extended care facility, please contact the social worker at the facility or the supervising nurse. They will coordinate discharge planning with the physician, nursing staff and therapist.

Privacy Policy
A copy of our HIPAA privacy policy is available on this website. This policy allows our staff to provide information to other providers, insurance companies and appropriate health care agencies. In addition to these, our office will only discuss the care of a patient in an extended care facility with the POA or family members who are directly involved in the patient’s care or payment for care. Other inquiries will be directed to the POA.

Hospital
Our physicians do not provide hospital care. We generally refer patients who need hospitalization to the Hospitalist Services at Carlisle Regional Medical Center, Holy Spirit Hospital, Pinnacle Health Hospitals, and other hospitals as desired or needed.
